Group and Personal Policies: Most plans provide for chiropractic care and will pay a percentage of costs. As a courtesy, our staff will verify your coverage and submit all claims, following up in every way possible to see that your insurance carrier meets their obligation for payment.

Managed Care Plans: Our office is a participating provider in many managed care plans in the area. Our staff is well qualified to answer any questions you may have about your HMO, PPO, or POS plans.

Personal Injury: We will file all necessary auto, accident, and health insurance policies for you. Dr. Rebarcak has testified as an expert witness in his field and has testified in court and in depositions on behalf of many patients.

Worker's Compensation: If you are injured at work, costs related to your care are usually covered 100% by your employer's worker's compensation carrier. We are experienced in the additional reports and forms needed for on-the-job injuries.

Medicare: We will submit your claims and in the event you have a supplemental policy, Medicare will usually forward that on to your insurance company. If your company is not one that they remit to, we will mail you a duplicate insurance form.

Medicaid: We are an affilitated provider with the Iowa Medicaid Title XIX program and will process all the forms and claims needed.
